Description
Expression of interest for an individual placement
We are looking for an experienced, specialist provider who can support an individual placement within a short time frame. The service provider should have extensive experience with supporting people with very high complex needs and able to provide the support in either a supported living environment (with accommodation) or residential provision.
Attached is a detailed pen picture of the person who requires support. However, as an overview the provider/staff must:
o Have specialist training and knowledge of Autism and Learning disability
o Experience in supporting people who display behaviours perceived as challenging
o Have specialist trained staff in PBS, high level restraint (as a last resort) de-escalation techniques
o Ethos of least restrictive practice
The person requires:
o a single occupancy dwelling (whether residential or supported living)
o level access accommodation (I.e. a bungalow), open plan with wet room
o semi-rural local ideally in or around the Stourbridge and Dudley area
Please read the attached specification thoroughly and if you feel you can meet that you could meet the person's needs, please provide an overview of how you are able achieve this on one side of A4 paper.
Shortlisted providers will be invited to undertake an assessment of the individual.
Providers must be CQC registered to provide either supported living services or residential care.
